Registered Dietitian

Buffalo, NY

GLIN IPA seeks a compassionate and knowledgeable Registered Dietitian to join our interdisciplinary outpatient care team. The Registered Dietitian will collaborate with providers, pharmacists, nurses, and social workers to deliver individualized nutrition counseling and develop network-wide protocols to improve health outcomes for patients with chronic medical conditions. The Registered Dietitian will support patients managing diabetes, prediabetes, obesity, malnutrition, and other nutrition-related conditions through evidence-based interventions. The responsibilities will include coordinating care, assessing risk factors, developing and negotiating personalized care plans, providing nutrition education (both group and individual), and monitoring patient progress while maintaining communication with healthcare providers.

Duties and Responsibilities (including but not limited to)

  • Assess the nutritional needs of the primary care network and identify areas where standardized protocols can enhance patient care.
  • Develop, implement, and evaluate nutritional protocols and workflows tailored to standard disease states, including diabetes, hypertension, and obesity.
  • Collaborate with primary care providers, pharmacists, and other healthcare professionals to integrate nutritional counseling into broader patient care plans.
  • Conduct initial and follow-up assessments to develop personalized nutrition and diabetes education plans.
  • Provide one-on-one and group counseling sessions focused on diabetes self-management, nutrition education, and lifestyle modifications.
  • Educate patients on carbohydrate counting, insulin management, glucose monitoring, and other diabetes-related skills.
  • Coordinate with multidisciplinary team members, including primary care providers, pharmacists, and other specialists, to optimize patient care.
  • Support patients in setting realistic goals, tracking progress, and overcoming barriers to adherence.
  • Participate in the development of educational materials and resources for patients.
  • Maintain detailed records of patient progress, documenting interventions and outcomes by GLIN IPA’s protocols.
  • Stay current on research and best practices in nutrition and diabetes education to improve patient outcomes continuously.

 

Qualifications

 Education:

  • Current registration as a Registered Dietitian (RD) with the Commission on Dietetic Registration (CDR) is required.
  • Certification as a Diabetes Care and Education Specialist (CDCES) by the Certification Board for Diabetes Care and Education (CBDCE) is preferred.

 

Experience (Minimum Needed):

  • A minimum of two (2) years of experience in nutrition counseling, focusing on diabetes education, is preferred.
  • Strong communication and interpersonal skills, with the ability to motivate and educate patients effectively.
  • Familiarity with electronic medical record (EMR) systems.
  • Knowledge of the Nutrition Care Process & Malnutrition Diagnosing Criteria.
  • Experience writing nutrition care plans for patients on enteral and parenteral feeds.
  • Proficient in discussing lab results with patients.
  • Familiarity with vitamins, minerals, and supplements.
  • Strong leadership skills and ability to work independently.

Knowledge and Skills/Expected for Performance of Job

  • A foundational understanding of HEDIS and STAR quality measures is helpful for this position, although not required.

Pay range $75,000 to $85,000

The referenced pay range represents the minimum and maximum compensation for this job.  Individual annual salaries/hourly rates will be set within the job’s compensation range and determined by considering factors including, but not limited to, market data, education, experience, qualifications, and expertise of the individual, as well as internal equity considerations.
